The Care You Need with a Little Extra TLC

Our Palliative Care Program is specialized medical care focused on providing relief from the symptoms and stress of a serious illness. The goal is to improve the quality of life for both the patient and the family by working with the patient’s primary care physician to provide an extra layer of support. It is appropriate at any age and at any stage in a life-limiting illness and, unlike hospice care, can be provided along with aggressive treatment.

We offer many services to assist you and your family in managing your care.

Physician Consultations

Our physicians are here to help with palliative care needs and symptoms.  Palliative Care specialists are offered through the home program.  They will work with you to create a plan of care focused on meeting the challenges of serious illnesses.

Advanced Care Planning

Opportunity to have discussions with trained healthcare professionals about future health choices based on your personal values, beliefs, and preferences.

In-home Social Work Needs Assessment

Comprehensive assessment of needs performed in the comfort of your home by our Palliative Care Social Work Navigator.

Care Coordination

Our team will actively work with you and your family to help coordinate medical equipment, home health services, and social services.

Medication Reconciliation/Counseling

A comprehensive medication review performed by our Clinical Pharmacist to ensure there is the most accurate list possible of all the medications you are taking to reduce medication errors.
