Patient-Centered Care at Health-e-Care 

While we offer basic primary care services for patients of all ages, our medical practice specializes in helping people with chronic conditions. We partner with primary care physicians and other medical specialists to support the appropriate management of chronic conditions. This partnership ensures that patients with chronic conditions always have their short-term and long-term health appropriately monitored. We also serve as after care monitors for people who have recently been hospitalized, providing follow-up services and coordinating on behalf of patients with other medical providers.
